Medical
SOURCECORP offers two BlueCross BlueShield medical options designed to cover a wide variety of medical services, including preventive care, office visits, prescription drugs, and inpatient care. The plans differ in how much you contribute for coverage and the level of benefits they provide.
1.  PPO (Preferred Provider Organization) Medical Plan - With the PPO plan, you have the freedom to choose either an in-network or out-of-network provider each time you receive medical care. Services provided by in-network providers are covered at a higher level than out-of-network providers.
2.  EPO (Exclusive Provider Organization) Medical Plan - The EPO plan covers only those services you receive from network providers. You and your covered dependents must use EPO network providers (except in an emergency away from home) to receive plan benefits. If you use providers outside of the network, your care will not be covered. The EPO plan offers lower annual deductibles and lower out-of-pocket expenses than the PPO plan.

Dental
The MetLife Preferred Dentist Program (PDP) includes more than 110,000 participating dentists nationwide. Benefits vary based on whether you receive services from a participating or non-participating provider.
 
Vision
Vision care services are offered through either EyeMed Vision or VSP Vision. EyeMed provides affordable quality vision care nationwide. The EyeMed network includes Pearle Vision, Lens Crafters, Sears Optical, Target Optical locations and participating Doctors of Optometry. VSP provides vision care through an extensive network of private-practice optometrists and ophthalmologists that dispense eyeglasses and contact lenses.

Company-Paid Basic Life and Optional AD&D Insurance
MetLife Basic Life Insurance is provided at no cost to you. You may buy additional protection through Accidental Death & Dismemberment insurance.

Employee Supplemental Life Options
In addition to Basic coverage, you can elect MetLife Supplemental Life Coverage in increments equal to 1x your annual salary, to a maximum of 5x (not to exceed $500,000).

Dependant Supplemental Life Options
MetLife insurance coverage is also available for your spouse and eligible children when employee supplemental life insurance is purchased.

Disability
SOURCECORP offers both Short Term Disability and Long Term Disability coverage through CIGNA.

Flexible Spending Account (FSA)
You can reduce your income taxes by contributing to Health Care & Dependent Day Care FSA accounts. Deposits into FSA are not subject to income or Social Security taxes.

Retirement Savings Plan (401k)

The SOURCECORP 401(k) Retirement Savings Plan, administered by Fidelity, allows you to contribute pre-tax earnings to an account whose earnings are not taxed until withdrawn.  To encourage employees to save for retirement, SOURCECORP has adopted an automatic enrollment plan.  As the name implies, after 30 days of employment payroll will automatically deduct a certain percentage of your pre-tax earnings (4% in 2009) and contribute to your 401(k) account.  You may elect out of this automatic deferral within the first 30 days of your employment.  At any time, you may elect to increase or decrease your deferral percentage within a range of 1% to 50%.  However, the total amount you may defer each year is limited.  For 2009, you may contribute up to $16,500 in your 401(k) account.  If you attain the age of 50 by the end of 2009, you may elect to contribute a catch-up adjustment up to $5,500.   You may only make contributions on eligible compensation.  Under the SOURCECORP plan, eligible compensation is all compensation reported as W-2 wages, but limited to $245,000 (for 2009).

 

Education Assistance Reimbursement

SOURCECORP recognizes the value of continuing education and professional development.  All full-time employees who have completed 12 consecutive months of employment are eligible to participate in the Educational Assistance Reimbursement.  The details of this policy are included in the employee handbook.

 

Employee Assistance Program

As part of our employee benefits package, SOURCECORP offers an EAP provided by CIGNA.  This program is designed to assist employees and their household members in obtaining help for a broad range of matters.  This company-paid, confidential program includes assistance such as legal services, financial planning assistance and discounts on many ancillary & alternative medical products.
